Signup form for UK Proxy

If you can just fill in this form below I will be able to get you up and running with the proxy and send you the required password.  During normal hours I can usually have you set up in less than an hour but at weekends I may take longer. NB you should get a reply email almost immediately if not then there is a chance it has fallen into your spam folder, if so you will need to add my email to the safe list otherwise you may miss the email with your details, which will arrive a little later.
